If you are travelling abroad, you may need to be vaccinated.
Please complete our online form below with as much information as you can. Our practice nurse will then review your completed form and your medical record.
Following this review, you will receive an email with all the information about what vaccinations are recommended and any other guidance about where you are visiting. You will then need to contact us to book an appointment for these travel vaccinations.
It is important to complete the form in as much details as you can so we can best advise you. Please make sure to complete and submit the form to us at least eight weeks before you travel so that the vaccines can be ordered. Your vaccination appointment should be at least two weeks before you travel to allow the vaccines to work.
The vaccines available at the surgery given by the NHS are diphtheria, tetanus, polio, MMR, hepatitis A, and typhoid. Any other vaccinations required can be given at a designated pharmacy or travel clinic. You will be notified of these in the correspondence from the surgery.
